Details:

• Full scope Optometry private practice
• 4 or 5 day work week available 
• One late day per week and half day on Fridays
• No weekends
• Great in house mentorship for new grads
• See 16 - 20 patients a day on average 
• Updated technology including Optos, OCT, Visual Field
• Great local relationships for co-management of patients
• Less than 20 minutes from Lake Winnebago

Compensation and benefits:

• $60-65/hr or 13% of receipts including optical sales
• $5,000 sign on bonus
• Relocation assistance negotiable
• IRA 3% match 
• 3 weeks PTO plus 6 paid holidays
